New Patients

We are pleased you have decided to come to Las Vegas Clinic for your medical care. We recommend you arrive roughly 20 minutes prior to your scheduled appointment to fill out necessary paperwork. Alternately, you may print and fill out the intake paperwork prior to your arrival.

You will need to provide a form of photo identification, as well as your current insurance information at the time of check-in. If your insurance requires you to make a co-payment, this will be collected at the time of check-in.
